Clinical Quality Measure - Controlling High Blood Pressure

This measure requires adding a Hypertension diagnosis to the Problem List for appropriate patients, and recording Blood Pressure in the Vitals List.

Click on the Lists Tab and highlight Problem List - click Edit 

The Problem List Administration window will open. Click on Problem Selection and type Hypertension for the options. 
There are a large number of Hypertension diagnosis codes, but the most common we see is I10.

All patients should have their blood pressure recorded in the Vitals List each office visit, including those with or without a Hypertension diagnosis.

Vitals may be recorded directly in the Vitals List, from the Office Visit Note, or from the Face Sheet.
