Practices that administer Influenza immunizations should record them in the Immunizations List.
Some practices record these in Injections, but that doesn't include all the data.

Detailed instructions for maintaining the Immunization inventory and recording immunizations for the patient are found here - Lists - Immunizations

Patients may have received the Flu vaccine from another provider, or they may choose not to get this immunization for a number of reasons.

If the patient has received it elsewhere then record the information in Health Maintenance or in the Immunization List.

There are a number of permitted exclusions for this measure. If patients did not receive a Flu vaccine for any reason it should be recorded in Health Maintenance. Indicate the reason in the Comment column.  Denominator Exception: Influenza immunization was not administered for reasons documented by clinician (e.g., patient allergy or other medical reasons, patient declined or other patient reasons, vaccine not available or other system reasons)
