Clinical Quality Measure - Tobacco Use: Screening and Cessation Intervention

The best process for meeting this measure involves 3 steps. 1. Record the Patient Smoking status in the eMedRec Social History List. Edit and Select the appropriate Smoking Status.

2. If the patient is a Smoker record Diagnosis Code Z71.6 in their Problem List. This will record the brief tobacco counseling activity in the chart.

3. After the Z71.6 code is added to the Problem List you can Right Click on the line for this diagnosis and select Patient Education.

4. Click Print and print the Smoking Cessation document created by the American Heart Association for the patient. This action is recorded in the database.

Following the process above will meet the CSM138V5 Clinical Quality Measure.
