The best process for meeting this measure involves 2 steps.
1. Record the Patient's Blood Pressure in the Vitals List.

2. If the physician has determined that the patient should be diagnosed with Hypertension then that diagnosis is added to the Problem List. Although there are other Hypertension ICD-10 codes, this examples shows a diagnosis of I10 - Essential hypertension primary.
